TEHRAN, Iran: A recent systematic review by Iranian scholars has compared implant treatment outcomes of over 1,500 patients who had undergone either minimally invasive surgery or standard surgery. Although the researchers discovered an important difference between the methods regarding the level of postoperative pain, they concluded that the general clinical outcome was not determined by the choice of surgical technique.
This study carried out at the Shahed University, Islamic Azad University Isfahan (Khorasgan) Branch and Shahid Beheshti University of Medical Sciences aimed to systematically review and compare the implant treatment outcome of the flapped and flapless techniques of implant insertion, including success and survival rates, marginal bone loss and postoperative pain. For the review, the researchers found 32 articles that met their inclusion criteria. In total, data on over 1,500 patients and 3,000 implants placed in maxillae and mandibles was included.
The study found that the implant survival rates of both surgical techniques were comparable. Furthermore, the occurrence of marginal bone loss was similar between the two methods, with 12 studies reporting higher marginal bone loss in the flapped groups and two in the flapless one. Oedema did not appear to occur to the same extent when using flapless surgery, although there was no statistically significant difference between the methods.
The only parameter that was statistically significantly affected by the choice of surgical technique was postoperative pain, which was reported less frequently after flapless surgery. In light of the results, an author at Dentsply Sirona noted that the patient’s well-being and experience is just as important as achieving outstanding overall results to ensure an optimal implant treatment outcome.
The study, titled “Dental implant placement with flapless and flapped technique: A systematic review”, was published online in the August/September 2018 issue of the Journal of Oral Research.
